What can I Expect During Botox Treatment?

Before Botox treatment, we will clean your skin thoroughly, and Dr. Sherber will ask you to move your facial muscles to make various expressions so that she can map out your muscular anatomy. The injections are not painful but you may feel a small pinch in more sensitive areas. Several points will be injected, with the exact number of injections needed determined by your anatomy and by the extent of the areas being treated. Treatment takes about 5-10 minutes.

It may look like your skin has small “bug bites” for a few minutes post-treatment, but Dr. Sherber will apply a soothing serum and gentle sunscreen to your skin before you depart to help calm and protect it.

Dr. Sherber often starts with a lighter dose with a new or first-time patient so as to ensure that the result is not overly dramatic. After two weeks, you would return for additional treatment if needed.

What is “Skin Botox” or “Microinfusion”

First developed in Singapore and also known as “Botox Facial,” this is not the traditional type of neuromodulator treatment common in the U.S. that will relax muscles to soften expression lines. Rather, Dr. Sherber will make a dilute sterile solution of the neurotoxin and stamp it into the dermis – the skin’s collagen layer – with tiny hollow needles rather than injecting into the muscles. This is a full-face treatment, and depending on where the Botox microinfusion is concentrated we can achieve diminished oil production and smaller pores, improvement in skin redness, and increased elasticity.

While some have tried administering filler in this way, research shows that the hyaluronic acid filler particles cannot pass through such small short needles; instead, to avail us of the skin’s increased absorption post-treatment Dr. Sherber will apply a smaller molecule form of hyaluronic acid topically that is the same type used in the Juvéderm fillers plus a customized cocktail of antioxidant vitamins.

Effects will have a comparable duration to traditional Botox treatments: 3-4 months. Topical numbing is not required but can be applied per your preference if you arrive 20 minutes before your appointment.

How does Botox work?

Repeated muscle contractions – such as frowning, squinting, or raising eyebrows – cause skin to furrow and fold gradually resulting in the formation of… facial lines. BOTOX® Cosmetic works beneath the surface and temporarily reduces the underlying muscle activity that causes moderate to severe frown lines, crow’s feet and forehead lines in adults to help them look visibly smoother.

Is BOTOX® Cosmetic FDA Approved?

BOTOX® Cosmetic was first approved by the FDA in 2002, and is the first and only treatment FDA-approved to temporarily make moderate to severe frown lines, crow’s feet and forehead lines look better in adults. It is used off-label in other areas, and the injectable can also be used to diminish unwanted perspiration in the underarms. Botox has been used safely with extremely high patient satisfaction for over more than 25 years.

How should I prepare for treatment?

Bruising is uncommon, but the risk can be minimized if you avoid non-steroidal anti-inflammatory medications (such as Aspirin, Advil, Aleve, Motrin, and Excedrin) for two weeks unless medically necessary, as well as any supplements containing fish oil, omega 3 fatty acids, ginkgo, ginseng, or ginger. If you have a history of herpes simplex cold sores, and if the injectable treatment is used near your mouth, we may prescribe an antiviral to take as an adjunct to treatment to prevent an outbreak. Botox treatment is not advisable if you are pregnant or nursing, so please let us know about this when scheduling so that we can discuss the best timing of your treatment.

Other Uses of Botox

About Your Washington DC Dermatologist

Dr. Sherber graduated magna cum laude from Harvard University, where she designed and implemented a novel degree program in health policy focused on assessing and improving quality of care. She next attended the Johns Hopkins University School of Medicine where she earned her M.D. degree. Following a post-doctoral fellowship in clinical research at Columbia University, she returned to Johns Hopkins for dermatology residency training and conducted research in scar healing and in disorders of abnormal collagen. She is now a Clinical Assistant Professor at George Washington University in the Department of Dermatology.
