Overview of Eyelid Lift Surgery in Washington DC

Men and women are often vexed by a fatigued, sad or disinterested appearance caused by loose upper eyelid skin, undereye bags, dark circles around the eyes, loose wrinkled undereye skin and hollowed eyesockets. Eyelid lifts, otherwise known as a blepharoplasty, are procedures designed to brighten the eyes and to convey a rested appearance. Upper eyelid lift focuses on removing excess heavy upper lid tissues that may cause the eyes to appear sad. Lower eyelid lifts are focused on removal of under-eye bags and filling the tear trough to give the eyes a rested appearance.

Dr. Rad also offers “Cat Eye” reshaping, or canthopexy, which involves subtle changes in the shape of the eye to lift the outer corners to correct situations where the outer eye may tilt downward. When performed expertly, blepharoplasty surgery (eyelid lift) can refresh the eyes. Dr. Rad’s approach focuses on natural results so that patients appear rested and with hidden scars tucked within natural creases or completely invisible on the inside of the eyelid surface. Combined upper and lower blepharoplasty can often safely be performed simultaneously. These tried-and-true approaches are safe and effective in experienced hands, and Dr. Rad is expert in achieving patients’ goals while minimizing recovery time.

Am I a Good Candidate for Eyelid Lift Surgery?

With age, the eyelid and eyebrow tissues sag, and under-eye bags appear. Although you may feel energetic and vibrant, your eyes may be lackluster and show fatigue or disinterest which can impact performance in the workplace and in professional and social settings. Eyelid lifts (blepharoplasties) are very effective to reduce signs of fatigue and optimal candidates are healthy with no eye or retinal disease, glaucoma or vision problems. Men and women with heavy upper eyelid skin and/or under-eye bags with deep tear troughs see the best results. The goal is a refresh, not looking like another person so that your outward appearance reflects your energy, drive and focus. If you feel like your upper eyelids are starting to sag, or you’re displeased with dark circles and bags under your eyes, you might be a good candidate for upper or lower eyelid lifts.

How Does Dr. Rad Approach Eyelid Lift Surgery?

Upper Blepharoplasty is an in-office procedure designed to contour the upper eyelids. It is safe and comfortable under local anesthetic. While resting quietly, the procedure lasts an hour and, in Dr. Rad’s hands, is completely painless. Small adhesive strips remain in place for 5-7 days and then are removed with sutures. Recovery is painless and bruising is mild. Patients can reasonably expect to return to work in 7 days.

For lower eyelid surgery, Dr. Rad performs this under general anesthesia which is delivered by a board-certified anesthesiologist in an accredited surgery center. He uses an invisible scar to remove the fat pads from behind the lower eyelids. Dr. Rad places an incision on the inside of the eyelid, a very safe method of scarless eyelid surgery. Oftentimes he combines this with Facial Fat Grafting in order to restore volume to deflated cheeks and under-eye regions.

For the Cat Eye reshaping, Dr. Rad places an upper eyelid incision within the natural fold and permanent sutures are placed to reposition the corners of the eyes slightly upward.

Q+A

WHAT NON-SURGICAL OPTIONS DO I HAVE?

Collagen-stimulating laser treatments can restore skin elasticity and, when combined with a daily eyelid-focused skincare regimen, the need for surgery can be delayed. Thermage FLX, the newest platform for skin tightening, is the only effective non-surgical device for reliable collagen contraction. Injectable dermal fillers can offer immediate results to restore lost volume in the eyesockets and to fill sunken tear troughs with no downtime.

WHAT IS THE RECOVERY FROM EYELID LIFT?

Surgery is outpatient and you’ll experience little discomfort easily controlled on mild pain medicine. You’ll have very little need for strong pain medicine. Mild bruising and temporary swelling are normal and resolve quickly, typically over 3-5 days, depending on the extent of your procedures. Dr. Rad’s detailed pre- and post-operative preparations ensure that you can achieve results quickly and with minimal down time. While healing and recovery will be different for each patient, return to work or going to social outings about 5-10 days after surgery is common.

I HAVE HANGING UPPER EYELIDS – WHAT SHOULD I DO?

Upper blepharoplasty is one of the most satisfying procedures because it can be performed comfortably in the office under local anesthetic. Every touchpoint is optimized for comfort and Dr. Rad guides you through your procedure with great precision and equally great comfort. Gentle numbing medicine allows you to relax while Dr. Rad removes excess eyelid skin without pain. Bruising is mild and sutures remain in place for 6-8 days. After one week eyes look refreshed and you may enjoy results for many years.

I HAVE PUFFY UNDEREYE BAGS – WHAT SHOULD I DO?

Lower blepharoplasty (eyelid lift) is a very satisfying procedure to restore rested eyes. Puffiness of the undereye is caused by protruding fat pads which, with deep tear troughs, can create a tired appearance. A combination of fat pad removal or repositioning through a hidden incision on the inside surface of the eyed, called “transconjunctival” approach, along with targeted fat transfer to the tear trough, restores a completely rested appearance in most cases. Surgery is performed under light general anesthesia (similar to that used in colonoscopy) and as an outpatient. Recovery, as defined by feeling good about one’s appearance and return to exercise, is approximately 10-14 days.

I HAVE WRINKLED UNDEREYE SKIN – WHAT SHOULD I DO?

Wrinkled skin is best addressed with the “subciliary” lower blepharoplasty approach which involves an incision beneath lower eyelashes and “unfurling” of wrinkled skin, redraping and lifting. Often Dr. Rad also tightens the lower eyelid margin to optimize results and prevent ectropion, a potential complication when surgery is not performed precisely, depending on your unique anatomy. While recovery is somewhat longer than the transconjunctival approach, results can be excellent.
